What to Expect on Your First Visit

1. At the time you come in for your first appointment*, you will be asked to provide us with some general information about yourself and your condition. (If you prefer, you may print this form, fill it out beforehand, and present it when you come in to see us.)
2. Once you've completed this paperwork, you will meet with Dr. White to discuss your specific health condition or challenge.
3. Dr. White will be ask you some questions about your medical history.
4. X-rays may be recommended to rule more serious conditions and to determine the most effective treatment for you.
5. Dr. White will then discuss with you the treatment options available.
6. You will choose the treatment that best meets your specific circumstances and needs.
7. Prior to leaving our office, you will be given written instructions on activities and procedures that you can do at home to help with your condition.
8. You will be given and opportunity to schedule your next appointment.
